Make checks payable to "IAQA Phoenix Chapter." Mail your check to 1553 W. Todd Drive, Suite 201, Tempe, AZ, 85283 OR hand carry your check to the workshop. NOTE: ***The Phoenix Chapter does not accept credit card payments due to the high costs of processing time and fees.***

For clarification: When you RSVP you lock in your cost, regardless of paying via mailed check or hand carrying a check to the event. The Chapter guarantees a specific number of attendees to the Arizona Club. Unless you give 48-hour notice of cancellation, when you RSVP you are committing to payment regardless of your attendance, since the Chapter is charged for the space and for each prepared meal. Also, the cost per additional head is higher for walk-in attendees due to the premium fee the Chapter is charged, when we exceed our reserved attendance number.

To RSVP, please contact
Stephanie Denis (StephanieD@CSCeng.com)
Voice: 480-460-8334 or Fax: 480-460-8335 (please state reason for call.)

Please email your Phoenix Chapter Director, Derrick A. Denis, at "derrickdenis@csceng.com" with your ideas for 2008 workshop speakers or topics.

Continuing Education Credits:

Many certifying bodies require certificants to prove they are attending industry educational opportunities. For your benefit, the Phoenix Chapter provides you with a certificate of completion and a receipt. Beyond this, the Phoenix IAQA Chapter can apply for continuing education credits (CEUs) for use towards keeping a variety of your certifications current. For example, attending the Phoenix IAQA Chapter workshops will provide you with four (4) re-certification credits (RCs) applicable toward renewing any American IAQ Council certification.

If you belong to a group for which you wish the IAQA to obtain approval for CEUs, then email your chapter director at derrickdenis@csceng.com. Please provide the necessary information, i.e. organization contact information, the application, specific workshop requirements, timeline for application process, etc.
